New Patient Info
Appointment
Most of our patients are referred to our practice for specialized care by their dentists. However, we welcome referrals from patients and friends of the practice. Please contact our office by phone at (310) 473-2727.
First Visit
Please assist us at the time of your initial visit to the office by providing the following information:
- Your referral slip and x-rays from your referring dentist.
- A list of medications you are currently taking
- If you have dental insurance, please bring any forms or insurance cards
Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, hypertension, medications for blood thinners or bisphosphonates).
Please Note: All patients under 18 must be accompanied by a parent or guardian at the consultation appointment.
Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
